Rights and Responsibilities

Crossroads Family Practice values respect for the individual, openness, equality and fairness, fiscal responsibility, teamwork, adaptability, and partnerships. We encourage you to play an active role in helping us to meet your needs. You can do this by ensuring that you are aware of your rights and responsibilities as a patient.

Patient Rights

As a patient of Crossroads Family Practice, you have the right to:

  • Be treated in a kind and respectful way.
  • Receive care without discrimination as to race, age, religion, disability, gender, sexual orientation or national origin.
  • Expect that all information about your healthcare will be kept private and confidential in accordance with the law.
  • Be given information in a way you will understand and be encouraged to participate in making decisions about your care.
  • Accept or refuse any treatment based on the possible risks, benefits, and alternatives, as long as your decision will not harm others (for example, some contagious diseases), as set out by law.
  • Know the name of the doctor in charge of your treatment and the roles of people involved in your care.
  • Know if clinical students may be helping in your care, under supervision. You have the right to refuse if you do not wish to have students participate.
  • Know if your care may be involved in research and that at all times, you will be asked for consent and have the right to refuse without your care being affected.
  • Express your concerns and get answers to your questions without affecting your care.

Patient Responsibilities

As a patient you have responsibilities. Please make sure you do the following:

  • Be polite and respect other patients, staff and members of the healthcare team.
  • Respect clinic property and act in a safe and responsible way.
  • Keep your contact information current, including who to contact in an emergency. This includes giving your healthcare team the name of the person who will represent you if you cannot make decisions by yourself.
  • Make every reasonable effort to keep your appointments and provide 24 hours notice of cancellation if you cannot.
  • Give accurate information to your healthcare team to help them plan your care.
  • Follow the plan of care the best way you can which includes understanding the information given to you and asking questions if you do not.
  • Accept responsibility for the decisions you make about your treatment.
  • Understand and be responsible for all expenses not covered by MSI.
  • Be understanding that your healthcare providers treat many people, and sometimes the needs of other patients and families may cause the doctor to run behind schedule.
  • Understand that some tests and treatments may be medically or ethically unnecessary or inappropriate, and that your doctor will make that determination.
